3/13/2023 EnchantaVaughn Approved Behavior: Upon arrival, Jahsim greeted QMHP with welcoming gestures. Jahsim displayed a calm spirit and seemed to be happy that his family was there to support him during this time. When engaging mother in a discussion about behaviors since the last session mother reports Jahsim does not know how to accept responsibility for positive behaviors because he is used to displaying negative behaviors so much and causing him to get suspended from school for eloping and hitting the teachers. Behavior: Jahsim was able to express his feelings regarding different matters associated around his mental illness and how it affects him with completing daily tasks. Jahsim implemented responsible behavior and was given a thumbs up by mental health professional that he was doing well with these tasks. Jahsim continued to display hyperactive behaviors such as him jumping from the couch to the chair upon the ending of the session. Intervention: QMHP supported Jahsim involvement in in-home counseling. QMHP explored positive behavior options with Jahsim to support his willingness to make good decisions and have positive behaviors. QMHP reinforced Jahsimâs positive behaviors by implementing a positive reward system in the session. QMHP offered simple praises to Jahsim for exhibiting the appropriate behaviors during the session and encouraged him to do his best even when no reward is given. QMHP continued to establish a positive supportive relationship with the Jahsim. Intervention: QMHP utilized cognitive skills to give the Jahsim a chance to openly and honestly discuss his feelings about his current mental state. QMHP utilized active listening skills and was empathetic with the Jahsim as he vented about his day. QMHP will review and give feedback regarding ADHD and its causes and effect to well-being at home, as well as, his performance in the community. QMHP inquired what type of activities Jahsim was involved in. QMHP asked Jahsim if these activities were available for him at home so that he can practice and learn them on his own. Response: Jahsim was open to session and communicated openly with counselor. Jahsim verbalized that heâs able to focus better when it is quieter and more peaceful at home. Jahsim modeled good behavior and was appreciative of QMHP support and encouragement. Jahsim talked a lot today but seemed to be gaining clarity about his ADHD condition. Jahsim was happy that he could confide in QMHP and not feel judged. Jahsim recognized positive behavior methods that would be useful in school, home, and community as a way to develop skills needed to stabilize symptoms associated with ADHD. Response: Jahsim displayed full participation in the session, as observed. Jahsim was cooperative and was able to vent and express his feelings calmly about his day. Jahsim expresses his needs as it pertains to meeting his objectives and working on other personal home goals. Jahsim was able to conduct positive reframing skills with guidance from QMHP. Jahsim listed strengths that he finds rewarding and beneficial to his daily functioning life. Jahsim shared that he was excited for his future and everything he was accomplishing.
